Zusammenfassung

In der Palliative Care findet eine konzeptionelle Verschiebung hin zu alten und chronisch kranken Menschen statt. Vor diesem Hintergrund nehmen die Leitungen von Langzeitpflegeeinrichtungen eine Schlüsselposition ein. Sie müssen auf Fragen und Ansprüche der letzten Lebensphase konzeptionell reagieren und Projekte und Initiativen zur Integration von Palliative Care unterstützen und ermöglichen. Genau hier knüpft die Palliative-Care-Box von Curaviva Schweiz an. Sie stellt funktionierende Beispiele und Instrumente aus der Praxis zur Verfügung, die in der Umsetzung adaptiert und zu eigenen Instrumenten weiterentwickelt werden können.

Abstract

There is a conceptional shift of palliative care towards the elderly and the chronically ill. Against this background, the managers of long-term care facilities occupy a key position. They have to react to questions and expectations about the end of life in a conceptual way. Furthermore, they have to enable and support projects and initiatives to integration of palliative care. This is precisely where Switzerland’s Curaviva Care Box comes in. It provides working examples and practical tools that can be adapted and further developed into instruments of one’s own.
